Allah is merciful. If you convert to Islam and practice it fully your past sins will be forgiven. BUT you have to make sure that you don't repeat those sins. and yes you can worship Allah at home without going to the mosque. To be honest, nowadays most women worship Allah at home. And I am so glad to hear that there is a person who is willing to change for the better .. good luck :)
